  1. ACTIVITIES OF PERFORMANCE
    1. PERSONAL CARE ACTIVITIES
      1. Cleanliness, Hyg. & Appear. - Bathes, performs toilet and oral hygiene activities, and dresses as needed.
        1. Bathing - Obtains and uses supplies; doffs clothing; achieves bathing position; maintains bathing position; soaps, rinses, dries all body parts; leaves bathing position.
          • 1) Obtains supplies - Obtains supplies (shampoo, soap, adaptive equipment).
          • 2) Doffs clothing - Removes (doffs) clothing.
            • aa. Upper body - Removes clothing from upper body.
            • bb. Lower body - Removes clothing from lower body.
          • 3) Regulates water - Turns on water and adjusts the temperature.

        aa2a. 1st arm - Removes first arm from sleeve of garment.

        aa2b. Around back - Pulls the garment around the back of the body.

        aa2c. 2nd arm - Removes second arm from sleeve.

        aa3. Bra - Excluding fastening; removes bra

        aa3a. Straps - Slips arms out of straps.

        aa3b. Removes - Pulls bra away from body.

        > bb. Dons upper clothing - Dons upper body clothing (underwear, outerwear, shoes).

        bb1. Pullover - Puts on pullover garment (undershirt, sweater, full slip, etc.); arranges.

        bb1a. Grasps - Picks garment up and positions it properly.

        bb1b. 1st arm - Puts first arm through the armhole.

        bb1c. Up 1st arm - Moves the garment all the way up the first arm.

        bb1d. 2nd arm - Puts the second arm in the other armhole.

        bb1e. Up 2nd arm - Moves the garment all the way up the second arm.

        bb1f. Head & neck - Puts head through the neck opening.

        bb1g. Down to sides - Pulls the garment down, attending to all sides of the body.

        bb1h. Adjusts - Straightens the garment.

        bb2. Front opening - Puts on garments that open in front, have front fastenings (jacket, shirt, button sweater, etc.); adjusts; excluding fastenings.

        bb2a. Grasps - Picks the garment up and positions it correctly.

        bb2b. 1st arm - Inserts one arm in the appropriate armhole.

        bb2c. Around back - Brings the garment around the back of the body.

        bb2d. 2nd arm - Inserts the second arm into the remaining armhole.

        bb2e. Onto shoulders - Pulls the garment up onto the shoulders.

        bb2f. Arranges - Straightens the garment.

        bb3. Bra - Puts on bra, excluding fastening.

        bb3a. 1st strap - Puts first arm through strap.

        bb3b. 2nd strap - Puts second arm through strap

        bb3c. Straightens - Assures straight material.

        bb3d. Ends together - Brings two ends together.

> 6) Lower Body - Dons and doffs lower body clothing (underwear, outerwear, and footwear); excluding fasteners.

        > aa. Doffs lower clothing - Removes lower body clothing (outerwear, underwear, shoes)

        aa1. Pants/Underpants - Takes off pants; excluding fastening; takes off underpants.

        aa1a. Waist to hips - Lowers garment from waist to hips.

        aa1b. One foot - Removes first foot from garment.

        aa1c. Second foot - Removes second foot from garment.

        aa2. Shoes/boots - Excluding fastening (laces, Velcro, etc.); removes shoes from both feet.

        aa2a. Off heel - Takes shoe off of heel (both feet).

        aa2b. Off of foot - Takes shoe off rest of foot (both feet).

        aa3. Socks - Takes socks off of both feet

        aa3a. Ankle - Pushes/pulls sock to top of ankle (both feet).

        aa3b. Over heel - Pushes/pulls sock off of heel (both feet).

        aa3c. Off feet - Pushes/pulls sock until it is completely off of foot (both feet).

        > bb. Dons lower clothing - Dons lower body clothing (underwear, outerwear, shoes).

        bb1. Pants/Underpants - Puts on pants and/or underpants; arranges clothing; excluding fastening.

        bb1a. Picks up - Picks up garment and positions properly.

        bb1b. Holds open - Holds the garment open.

        bb1c. 1st foot - Inserts the first foot.

        bb1d. 2nd foot - Inserts the second foot.

        bb1e. Over feet - Pulls the garment over both feet.

        bb1f. Over knees - Pulls the garment up above both knees.

        bb1g. Pulls to waist - Pulls the garment to the waist.

        bb3b. Holds open - Holds sock open in preparation to insert foot (both feet).

        bb3c. On toes - Puts sock on toes (both feet).

        bb3d. Over heel - Pulls sock up over heel (both feet).

        bb3e. Up from ankle - Pulls sock up from ankle (both feet).

        bb3f. Adjusts - Adjusts fabric on foot (both feet).

> 7) Clothing accessories - Puts on and takes off clothing accessories such as hats, scarves, mittens,

        jewelry, watches, neckties, etc.; stores properly; excluding fastenings.

        aa. Dons - Puts on clothing accessories.

        bb. Doffs - Takes clothing accessories off.

        cc. Stores - Places clothing accessories in appropriate location.

> 8) Specialty clothing - Puts on and takes off specialty clothing such as tights, swimsuits, specialty dresses, costumes, etc.; stores properly; excluding fastenings.

        aa. Dons - Puts on specialty clothing.

        bb. Doffs - Takes specialty clothing off.

        cc. Stores - Places specialty clothing in appropriate location.

> 9) Assistive Device(s) - Puts on and takes off assistive device(s) which are worn (prosthesis, orthosis, glasses, splints, hearing aids, etc.); stores properly, excluding fastenings.

        aa. Dons - Puts on assistive device(s).

        bb. Doffs - Takes assistive device(s) off.

        cc. Stores - Places assistive device(s) in appropriate location.

        > g. Nose Blowing - Grasps tissue or handkerchief; blows and  wipes nose.

1) Grasps - Grasps tissue or handkerchief.

2) Wipes nose - Blows and wipes nose.
