- PERSONAL CARE ACTIVITIES
- Cleanliness, Hyg. & Appear. - Bathes, performs toilet and oral
hygiene activities, and dresses as needed.
- Bathing - Obtains and uses supplies; doffs clothing; achieves bathing position; maintains bathing position; soaps, rinses, dries all body parts; leaves bathing position.
- 1) Obtains supplies - Obtains supplies (shampoo, soap, adaptive equipment).
- 2) Doffs clothing - Removes (doffs) clothing.
- aa. Upper body - Removes clothing from upper body.
- bb. Lower body - Removes clothing from lower body.
- 3) Regulates water - Turns on water and adjusts the temperature.
- aa. Turns faucets on/off - Reaches and turns faucets on and off.
- bb. Regulates temperature - Sets appropriate temperature.
- 4) Gets in position - Gets in bathing position (transfers to shower, tub, or bathing seat).

> 4) Gets in position - Gets in bathing position (transfers to shower, tub, or bathing seat).

aa. Uses supplies - Uses washcloth, sponge, or mitt with appropriate amount of soap; able to handle soap.
bb. Face and neck - Lathers and rinses face and neck.
cc. Front torso - Lathers and rinses front half of trunk.
dd. Back torso - Lathers and rinses back.
ee. Front perineal area - Lathers and rinses front perineal area.
ff. Back perineal area - Lathers and rinses back perineal area, including buttocks.
gg. Left arm - Lathers and rinses left arm, including underarm.
hh. Right arm - Lathers and rinses right arm, including underarm.
ii. Left leg - Lathers and rinses left leg.
jj. Right leg - Lathers and rinses right leg.
> 6) Shampoos hair - Uses appropriate amount of shampoo to wash hair; rinses hair; (conditions if appropriate).
aa. Uses shampoo - Uses appropriate amount of shampoo or conditioner.
bb. Washes - Lathers hair and scrubs scalp.
cc. Rinses - Rinses soap/conditioner from hair
> 7) Dries self - Dries body, including hair.
aa. Face and neck - Dries face and neck.
bb. Front torso - Dries front half of trunk.
cc. Back torso - Dries back.
dd. Left arm - Dries left arm, including underarm.
ee. Right arm - Dries right arm, including underarm.
ff. Left leg - Dries right leg.

> 8) Leaves position - Leaves bathing position.
> b. Toilet Hygiene - Obtains and uses supplies; achieves toileting or elimination position; maintains
position; cleans; leaves toileting or elimination position.
> 1) Obtains supplies - Obtains and uses supplies (toilet paper, other supplies and devices).
> 2) Gets in position - Achieves toileting position (transfers to toilet, commode, or moves into position).
> 3) Eliminates - Produces/withholds eliminating contents as appropriate.
> 4) Cleans self - Cleans self (reaches and wipes anal and or genital areas sufficiently).
> 5) Leaves position - Leaves elimination position
(resumes ambulation position, replaces clothing correctly).
> 6) Flushes toilet - Flushes toilet.
> 7) Menstrual needs - Manages menstrual needs; applies, removes device.
> c. Hand Washing - Obtains and uses supplies; regulates water; soaps, scrubs and rinses, shuts water off; dries hands.

> 2) Turns on water - Turns on water and adjusts the temperature.
aa. Turns faucets on - Reaches and turns faucets on.
bb. Regulates temperature - Sets appropriate temperature.

> 5) Dries hands - Dries hands with towel, electric dryer.
> d. Oral Hygiene - Obtains and uses supplies; cleans mouth and teeth; rinses pertinent to natural teeth or dentures.
> 1) Obtains supplies - Obtains supplies (toothbrush, paste, dental floss, mouthwash).
> 2) Brushes - Brushes teeth (applies paste, brushes, rinses).
aa. Applies paste - Manages toothpaste container; applies appropriate amount.
bb. Brushes - Brushes all teeth.
cc. Rinses - Rinses toothpaste from mouth.
> 3) Mouthwash - Uses mouthwash (pours mouthwash into glass, rinses for appropriate amount of time, discards wash).
> 4) Flosses - Flosses teeth (obtains appropriate length of floss, flosses teeth, discards floss).
> 5) Stores dentures - Stores dentures in an hygienic and safe location.
> e. Grooming - Obtains and uses supplies; shaves; applies and removes cosmetics; combs and brushes hair.
> 1) Cares for hair - Obtains needed tools and supplies; combs/brushes; styles hair.

> 4) Fastens - Closes fasteners for all clothing and accessories (fasteners include buttons, hooks in back, snaps, zippers, hook and loop, laces,watchbands, ties, etc.)
aa. Upper body - Closes fasteners for all upper body clothing (fasteners include buttons, hooks in back, snaps, zippers, hook and loop, laces, etc.)

> 5) Upper Body - Dons and doffs upper body clothing (underwear and outerwear) excluding fastenings.
> aa. Doffs upper clothing - Removes upper body clothing (outerwear, underwear, shoes)
aa1. Pullover - Takes off garments that pull over the head (undershirt, sweater, full slip, etc.)
aa1a. Grasps - Grasps garment to remove.
aa1b. Pulls up - Pulls the garment up to chest height.
aa1c. Pulls over - Pulls garment over shoulders.
aa1d. Pulls off - Pulls the garment off of arms and away from body.
aa2. Front opening - Takes off garments that have front openings (shirt, button sweater, jacket, etc.); excluding fastenings.
aa2a. 1st arm - Removes first arm from sleeve of garment.
aa2b. Around back - Pulls the garment around the back of the body.
aa2c. 2nd arm - Removes second arm from sleeve.
aa3. Bra - Excluding fastening; removes bra
aa3a. Straps - Slips arms out of straps.
aa3b. Removes - Pulls bra away from body.
> bb. Dons upper clothing - Dons upper body clothing (underwear, outerwear, shoes).
bb1. Pullover - Puts on pullover garment (undershirt, sweater, full slip, etc.); arranges.
bb1a. Grasps - Picks garment up and positions it properly.
bb1b. 1st arm - Puts first arm through the armhole.
bb1c. Up 1st arm - Moves the garment all the way up the first arm.
bb1d. 2nd arm - Puts the second arm in the other armhole.
bb1e. Up 2nd arm - Moves the garment all the way up the second arm.
bb1f. Head & neck - Puts head through the neck opening.
bb1g. Down to sides - Pulls the garment down, attending to all sides of the body.
bb1h. Adjusts - Straightens the garment.
bb2. Front opening - Puts on garments that open in front, have front fastenings (jacket, shirt, button sweater, etc.); adjusts; excluding fastenings.
bb2a. Grasps - Picks the garment up and positions it correctly.
bb2b. 1st arm - Inserts one arm in the appropriate armhole.
bb2c. Around back - Brings the garment around the back of the body.
bb2d. 2nd arm - Inserts the second arm into the remaining armhole.
bb2e. Onto shoulders - Pulls the garment up onto the shoulders.
bb2f. Arranges - Straightens the garment.
bb3. Bra - Puts on bra, excluding fastening.
bb3a. 1st strap - Puts first arm through strap.
bb3b. 2nd strap - Puts second arm through strap
bb3c. Straightens - Assures straight material.
bb3d. Ends together - Brings two ends together.
> 6) Lower Body - Dons and doffs lower body clothing (underwear, outerwear, and footwear); excluding fasteners.
> aa. Doffs lower clothing - Removes lower body clothing (outerwear, underwear, shoes)
aa1. Pants/Underpants - Takes off pants; excluding fastening; takes off underpants.
aa1a. Waist to hips - Lowers garment from waist to hips.
aa1b. One foot - Removes first foot from garment.
aa1c. Second foot - Removes second foot from garment.
aa2. Shoes/boots - Excluding fastening (laces, Velcro, etc.); removes shoes from both feet.
aa2a. Off heel - Takes shoe off of heel (both feet).
aa2b. Off of foot - Takes shoe off rest of foot (both feet).
aa3. Socks - Takes socks off of both feet
aa3a. Ankle - Pushes/pulls sock to top of ankle (both feet).
aa3b. Over heel - Pushes/pulls sock off of heel (both feet).
aa3c. Off feet - Pushes/pulls sock until it is completely off of foot (both feet).
> bb. Dons lower clothing - Dons lower body clothing (underwear, outerwear, shoes).
bb1. Pants/Underpants - Puts on pants and/or underpants; arranges clothing; excluding fastening.
bb1a. Picks up - Picks up garment and positions properly.
bb1b. Holds open - Holds the garment open.
bb1c. 1st foot - Inserts the first foot.
bb1d. 2nd foot - Inserts the second foot.
bb1e. Over feet - Pulls the garment over both feet.
bb1f. Over knees - Pulls the garment up above both knees.
bb1g. Pulls to waist - Pulls the garment to the waist.

bb2d. 1st half - Puts half of foot into shoe (both feet).
bb2e. Over heel - Puts the rest of the foot into shoe (both feet).
bb3. Socks - Puts socks or onto both feet; adjusts (fabric)
bb3a. Picks up - Picks up sock and locates the topboth feet).
bb3b. Holds open - Holds sock open in preparation to insert foot (both feet).
bb3c. On toes - Puts sock on toes (both feet).
bb3d. Over heel - Pulls sock up over heel (both feet).
bb3e. Up from ankle - Pulls sock up from ankle (both feet).
bb3f. Adjusts - Adjusts fabric on foot (both feet).
> 7) Clothing accessories - Puts on and takes off clothing accessories such as hats, scarves, mittens,
jewelry, watches, neckties, etc.; stores properly; excluding fastenings.
aa. Dons - Puts on clothing accessories.
bb. Doffs - Takes clothing accessories off.
cc. Stores - Places clothing accessories in appropriate location.
> 8) Specialty clothing - Puts on and takes off specialty clothing such as tights, swimsuits, specialty dresses, costumes, etc.; stores properly; excluding fastenings.
aa. Dons - Puts on specialty clothing.
bb. Doffs - Takes specialty clothing off.
cc. Stores - Places specialty clothing in appropriate location.
> 9) Assistive Device(s) - Puts on and takes off assistive device(s) which are worn (prosthesis, orthosis, glasses, splints, hearing aids, etc.); stores properly, excluding fastenings.
aa. Dons - Puts on assistive device(s).
bb. Doffs - Takes assistive device(s) off.
cc. Stores - Places assistive device(s) in appropriate location.
> g. Nose Blowing - Grasps tissue or handkerchief; blows and wipes nose.
